CONCUSSIONS

Rec League Requirements:

Annually, each coach and assistant coach must complete the following:

• Complete online concussion training and print out certificate (see at bottom of page)
• Complete Concussion Information and Signature Form for Coaches (Click Here)
• Submit concussion training certificate and completed Concussion Information and Signature Form for Coaches to Coffee County Soccer League (CCSL) Board BEFORE any games begin for the season.

Parents/Guardians 

• Complete Concussion Information and Signature Form for Student Athletes and Parents/Legal Guardians 
• Submit completed Concussion Information and Signature Form for Student Athletes and Parents/Legal Guardians (Click Here) to the Coach, who will forward them to the CCSL Board.  This form must be completed BEFORE player will be allowed to play in any games.
